This is your opportunity to contribute to the biggest single report on what it is like to live as a family of a child with disabilities or additional needs in Calderdale in 2015.

When the survey closes Calderdale Parents and Carers will compile a detailed report which will:

  • Help to shape the way services are provided and influence local priorities
  • Be presented to senior managers and commissioners in Calderdale
  • Provide information to influence policy and approach

All feedback will be anonymous and your contact details will not be shared.

There will also be a drop in session on Tuesday 15 March from 1pm to 3pm at the Calderdale Parents and Carers office on Hanson Lane, where staff from the Advocacy service will be on hand to help you.

Alternatively you can schedule a telephone session with Amanda Rorrison by calling 01422 343090 or emailing info@cpandc.org.uk

Each completed survey will be entered into a prize draw to win one of 4 Marks and Spencers vouchers worth £50 – Click here to complete the survey online
