Future NHS provide translated materials and vaccination consent forms
The Future NHS website now provides translated materials page for the Covid Vaccine.
This includes UKHSA consent forms for vaccinating minors.
The consent forms have been translated into the following languages: French, Gujarati, Polish, Urdu, Tamil, Arabic, Punjabi, Farsi, Somali, Lithuanian, Nepalese, Portuguese, Spanish, Bengali, Turkish, Romanian, and Hebrew.
To access these you need to a be registered with NHS futures.
Future NHS is a useful resource, easy to use and very quick to register.

Long Covid Peer Support Survey March 25
Calderdale Council Public Health are interested in the experience of people with Long Covid to help identify what support is needed.
If you think you have had any symptoms of Long Covid, we invite you to please share your experience by completing our short survey.
This survey will take approximately 10 minutes to complete. The deadline for completion is March 25th 2022.
Your valuable feedback will help us to understand experiences of current Long Covid support and what is missing.

Last Chance to Have Your Say on changes to Hospital Services
The public consultation on proposed changes to hospital services and further improvements to community services in Calderdale and Greater Huddersfield will be closing at midnight on Tuesday 21st June 2016.
The Right Care, Right Time, Right Place consultation is being led by NHS Calderdale Clinical Commissioning Group (CCG) and NHS Greater Huddersfield CCG and supported by Calderdale and Huddersfield NHS Foundation Trust.

Join the Calderdale Vanguard Community Panel
Voluntary Action Calderdale are looking for people who are passionate about local health and social care services to join a Community Panel.
The panel will work alongside local providers (Calderdale CCG, Calderdale Council, the hospital and mental health trusts, GPs, Locala and the Third Sector) to discuss plans and ideas for new ways delivering quality services closer to home, in local communities.
